职位描述
Qualifications:
a. Bachelor's degree in pharmacy major or equivalent
b. Minimum 7 years work experience in a pharmacy(community or hospital) setting or 2year work experience in international clinic setting
c. Licensed Pharmacist License or Senior title by Ministry of Health required
Skills:
a. Be able to perform the above duties independently.
b. Excellent communication skills in English and/or Chinese.
c. Provide supervision and training to junior pharmacist.
Knowledge:
a. Knowledge of government laws pertaining to drugs, narcotics and pharmacy practice.
b. Knowledge of drug information in English and Chinese and familiar with drug formulary
c. Knowledge of clinical pharmacy preferred
Responsibilities:
Primary Duties-
a. Reviews and interprets prescription orders and verifies accuracy and completeness of patient labeling and input into pharmacy computer.
b. Evaluates and resolves real or potential prescription related problems.
c. Provides patient consultation on prescription and OTC medications.
d. Establish a pharmaceutical care practice in conjunction with physicians and other care providers.
e. Reviews inpatient orders and screens them for possible duplications, interactions.
f. Dispense medication to in-house patients
g. Actively participates in clinical pharmacy projects and quality indicator monitoring
h. Provides guidance and supervision for junior pharmacists and pharmacist trainees.
Other Duties Performed-
a. Ordering inventory and doing periodic inventory check.
b. Performing monthly narcotic inventory.
c. Take part in night and weekend shifts.
d. Compounding products for our department stock.
e. Ensuring and promoting medication use safety in hospital
