职位概述

Dispensing medication to patients and providing drug information to patients and hospital staff, performing clinical pharmacy projects. 向患者分发药物以及向患者和医院职工提供药品信息，执行临床药学项目。

岗位职责

Primary Duties-主要职责
1. Reviews and interprets prescription orders and verifies accuracy and completeness of patient labeling and input into pharmacy computer.
检查并解释处方，核实病人标签的精确性和完整性，并将其录入药房电脑
2. Evaluates and resolves real or potential prescription related problems.
评估和解决处方相关的实际问题和潜在问题。
3. Provides patient consultation on prescription and OTC medications.
向病人提供处方和非处方药的咨询服务。
4. Establish a pharmaceutical care practice in conjunction with physicians and other care providers.
协同内科医生和其他护理人员建立药学服务实践。
5. Reviews inpatient orders and screens them for possible duplications, interactions.
检查住院订单，为可能使用的副本或互动进行扫描。
6. Dispense medication to in-house patients
向内部病人分发药物。
7. Actively participates in clinical pharmacy projects
积极参与临床药学项目。
8. Provides guidance and supervision for junior pharmacists and pharmacist trainees.
向初级药剂师和药剂师学员提供指导和监督。
Other Duties Performed- 其他职责
1. Ordering inventory and doing periodic inventory check.
订货并定期检查库存。
2. Take part in evening and weekend shifts. Participate in night on-calls if needed.
参与晚上和周末的值班，以及假期换班。需要时参与晚间电话候命。
3. Compounding products for our department stock.
为部门库存复合产品。
4. Ensuring medication use safety in hospital
确保医院用药安全。

任职条件

1. Bachelor’s degree in pharmacy major or equivalent
药学专业本科学历或同等学历
2. Minimum 3 years work experience in a pharmacy (community or hospital) setting or 1 year work experience in international clinic setting
3年以上药房环境工作经验（社区或医院），或1年国际诊所工作经验。
3. Licensed Pharmacist License or Pharmacist title by Ministry of Health required
有药剂师资格证或经卫生部许可的药剂师。
