Position Summary:
Serve as the contact person for Jiahui Clinical Service Units to provide patient a smooth and pleasant patient experience within Jiahui system, and provide them necessary information about service, patient education and other patient benefit resources in Jiahui.
DUTIES AND RESPONSIBILITIES:
Coordination of patient care
- Be single point of contact and manage all aspects of new and existing patient queries
- Identify and develop relationships with personnel in departments involved in the care of patients.
- Facilitate new patients medical visit by reviewing records and applications in advance
- Coordinate services need to be provided include but not limited to arrange resources, set schedule, transportation and escort.
- Master services and be able to introduce and offer the correct services to patients and their families
- Assess treatment needs, develop, monitor, evaluate treatment plan and progress for cancer patients according to physician and nursing’s treatment plan
- Monitor cases by checking patients’ attendance; evaluating patients’ experience.
- Maintain patients’ records by reviewing case notes; logging events and progress
- Ensure appropriate health information and education about their diagnosis and treatment plan.
- Engage patients and their families to be part of the treatment process to the degree their desire
- Develop and responsible for patient support groups and patient educational workshops
Communication between patients and other healthcare professionals
- Develop effective working relations and cooperate with other healthcare professionals and patients/their families throughout the entire case management process to ensure the achievement of quality care and best possible outcomes for patients
- Participate and communicate patients’ progress in interdisciplinary meetings
Support patients financial decision making process
- Assess the needs of patients to enable the most appropriate provision of services in a timely and cost effective manner
- Provide financial counselling for all patients; initiate and follow up GOP requests
- Enhance and achieve new patient treatment conversion and existing patients’ retention target set by service units
- Develop plans to optimize and improve conversion ratios
Others
- Support marketing by completing surveys
- Promote hospital public relations
- Adheres to professional standards, policies and procedures, quality/safety standards; updates job knowledge by continuous learning; exploring opportunities to provide value added services to patients
- Demonstrate profession’s clinical standards and Code of Ethics
- Ability to navigate systems with patients/families. Share knowledge of community agencies and resources.
- Demonstrate initiative and leadership in developing services which respond to patient care needs.
?EXPERIENCES, SKILLS AND COMPETENCIES:
- Nursing/medical background must to have
- Proven experience in customer service
- Has practical knowledge of different type of cancers, procedures and treatments specific to service
- Strong verbal and written communication with all stockholders
- Strong assessment skills
- Excellent organizational skills
- Computer literacy to maintain and manage case records
- Clinical assessment and analytical skills
- Being able to multitasking effectively
- Excellent time management skills
- Critical thinking and Problem solving skill
?LANGUAGE PROFICIENCY:
Chinese and English speaking
- Reading/writing medical records and documents in Chinese and English
- Basic oral communication with patients and other staffs in Chinese and English
- Additional languages like Japanese/Korean are desirable
EDUCATION:
- Bachelor degree in nursing or above
