POSITION SUMMARY:
Under general direction of the senior pharmacist/pharmacy director, the pharmacist performs a variety of professional level functions to a key member of the Pharmacy Team assisting in the provision and development of a comprehensive, high standard, safe and efficient pharmacy service which consistently meet the needs of all service users within the hospital.
KEY TASKS AND RESPONSIBILTIIES:
Dispensary service
1. Accurately identify, select, label using HIS, dispense and distribute drugs (including controlled drugs) for in and outpatients prescriptions according to Standard Operating Procedures (SOP’s).
2. Take part in counselling and in the provision of information on medicines to patients, doctors and other health care staff.
3. Carry out reception duties, involving dealing with patients, relatives and handling prescription charges/crediting unused medicines.
4. Use the computer systems to track prescriptions. Ensure medication is dispensed to meet agreed delivery times and to minimize waiting times for patients.
5. Deal with queries from patients and other hospital staff, referring complex issues to more senior staff.
6. Accuracy check of dispensed items.
7. Ensure that all medicines are stored according to environmental requirements (e.g. temperature) and to required security standards.
8. Ensure stock levels of all products are accurate and support the appropriate ordering of stock through liaison with pharmacy purchasing and procurement office.
9. To assist in the documentation and preparation of emergency drug boxes.
10. To complete drug expiry date checking.
Purchasing and Stock Control
1. Assist with basic purchasing, maintenance and control of stock levels for all facilities using purchasing and inventory systems.
2. Co-ordinate and assist in receiving Pharmacy stock (medical and non-medical) against the orders placed for all Pharmacy locations.
3. Follow the procedures for checking, storing and handling each medicine received including dealing with discrepancies found in deliveries.
4. Assist with any invoice queries arising.
5. Co-ordinate and assist in accurately inputting data on the Pharmacy computer system for all deliveries received and recording delivery anomalies/any other required audit records. To be involved in stock rotation, cycle counts and routine expiry date checks both within the pharmacy and on the wards.
6. Maintain a clear system for filing order sheets / paperwork as per procedures.
7. Receive unlicensed and special drugs and ensure correctly entered on the Pharmacy systems.
8. Liaise with suppliers by telephone with queries regarding delivery of urgent medicines, non delivery of medicines and to chase uncompleted or delayed orders in a timely manner. To assist with stocking and maintenance of intravenous fluid store.
9. Support the section leaders in monitoring supply problems, investigating resolutions, and cascading supply problems to the pharmacy group.
Clinical service
1. Provide an efficient and accurate ward top-up service according to hospital policy.
2. Be responsible for ADC re-stocks, stock allocation and expiry checking.
3. Check expiry dates and stock holdings on the floors/departments on a regular basis.
4. Regularly audit ward treatment room drug storage compliance.
5. Ensure all medicines for new admissions are assessed and reconciled in line with local policy, liaise with external organisations.
6. Accurately and promptly record all patient histories electronically.
7. Accurately assess Patient Own Medicines in line with local policy to ensure they are of a suitable standard and quantity for use. Encourage the use of these medicines at ward level.
8. Accurately order and replenish prescribed medicines that have been clinically checked by a pharmacist to ensure sufficient quantity during the patients stay.
9. Coordinate patients’ discharge with the ward team identifying patients for discharge.
10. Accurately check discharge prescriptions against the discharge summary sheet to ensure they have been transcribed correctly by the prescriber before sending to pharmacy for dispensing and reconciling using existing PODs and inpatient supplies.
11. Accurately communicate any medicine related issues that require follow up to the ward team using the appropriate communication channel.
12. Effectively counsel and give advice to patients in order to facilitate the safe and effective use of medicines using the agreed procedure.
13. Responsible for ensuring Controlled Drugs which are no longer required for use on the ward are returned or destroyed as per local policy.
14. Responsible for implementing pharmacy training programmes for a broad range of technical/pharmacist staff who work across the Jiahui Health.
15. Review and liaise with the pharmacist to update ward stock lists in line with ward usage each month. Provide data on any cost savings made to the ward stock lists to the pharmacy management team.
PIVAS service
To participate in aseptic preparation and manufacturing in accordance with Good Manufacturing Practice, COSSH regulations, current legislation/guidelines and policies and standard operating procedures (SOPs). This involves:
§ The use of specialist equipment including working in clean-rooms and the use of isolator cabinets
§ Undertaking the handling, manipulation and accurate measurement of all component drugs.
§ Ensuring strict asepsis at all times within the work-station.
§ Documenting the completion and accuracy of critical stages in aseptic manufacture and preparation.
§ Completing all in a safe, accurate and timely manner.
§ Assembling, calibrating, dismantling and cleaning after use: all equipment.
§ Visually checking, labelling and packing of products prepared during the session.
§ Recording all deviations in the process and informing senior staff of the deviation.
§ Identifying and reporting of actual and potential errors.
§ Recognising limitations in the process and own limitations and referring immediately to senior staff.
§ To participate in the operation and maintenance of systems for:
1. Prioritisation and scheduling of work.
2. Distribution of completed work, ensuring quality of the product and safety of staff, visitors and patients.
3. Documentation and documentation control.
4. Monitoring of function, state of cleanliness and state of repair of all equipment and facilities.
5. Cleaning of the isolators, equipment and aseptic facilities.
6. Microbiological and environmental monitoring within the unit.
7. Cost effective use of resources including stock control of products, raw materials, drugs and equipment.
8. Undertake the required validation or re-validation exercises: to enable the operator to work satisfactorily within all areas of Aseptic Preparation and other areas of the Unit.
9. Ensure manufacturing work sheets are completed accurately and fully for each product made if required using the systems.
