岗位职责:

Summary of Responsibilities:

Responsibility for the care management of patient populations (Unity Health members and other populations) identified in partnership with a core team and key care providers

By Helping to:
- Analyze the particular needs of these populations by looking at top diagnoses
- Stratify patients into appropriate tiers of care requirements – e.g. self-care, chronic, poorly controlled, intensive
- Collaborate in developing priorities, patient goals and care plans
- Be active in setting up patient education processes and materials
- Develop outcome metrics e.g. condition metrics’ improvements. patient satisfaction, #s of visits/utilization rates

Carry out hospital policies and procedures:
- To safeguard patient wellness and autonomy through the coordination of all care and available resources and through the promotion of patient advocacy.
- Coordination of patient-focused care in a cost-effective manner without diminishing the quality of care a patient receives.

任职资格:

? Bachelor’s degree in nursing or allied medical field
? Minimum Experience Requirement – 5 years work experience in nursing, capable of taking care patients across diagnoses, Previous experience in Case management desired
? Bilingual English & Chinese preferred
